Step-by-Step Instructions

Click here to enter         Unilogic Healthcare Ticketing

  • If you haven’t registered, fill out all required fields to create an account

  • If already registered, log in using your credentials​​​

Register or Log In

  • Click New Ticket

  • Select Category

    • Choose Agent/FMO Help Desk​

  • Enter Subject Line​

    • ​Use: “Membership Validation”​

Submit a New Ticket

  • Upload your membership list file (Word or Excel format preferred)

Ensure the list includes:

  • Member full name

  • DOB

Attach Membership List

  • Health plan

  • Member ID

  • Add Description

    • Briefly describe the purpose of the submission         

  • Click Submit and wait for Eligibility team confirmation within 3–5 business days.
