Select Category

Choose Agent/FMO Help Desk​

Enter Subject Line​
-
​Use: “Membership Validation”​

Submit a New Ticket

Upload your membership list file (Word or Excel format preferred)
Ensure the list includes:

Member full name
-
DOB

Attach Membership List
-
Health plan
-
Member ID
-
Add Description
-
Briefly describe the purpose of the submission

Click Submit and wait for Eligibility team confirmation within 3–5 business days.
