Position Summary
The Chronic Condition Wellness Program Specialist is a non-clinical professional responsible for supporting individuals managing chronic conditions such as diabetes and high blood pressure. This role focuses on patient advocacy, health education, resource navigation, and program coordination to empower members in improving their overall well-being. The ideal candidate is passionate about community health, chronic disease prevention, and helping individuals overcome barriers to wellness.
Essential Responsibilities
Member Support & Wellness Coaching:
Provide guidance and support to members managing chronic conditions by connecting them with appropriate wellness resources.
Conduct outreach and follow-up with program participants to encourage engagement and adherence to wellness plans.
Educate members on lifestyle changes, nutrition, and self-management strategies for diabetes, hypertension, and other chronic conditions.
Assist members in navigating non-clinical healthcare resources, social services, and wellness programs.
Help members set achievable wellness goals and track progress over time.
Advocate for members by addressing barriers such as transportation, language, financial concerns, or technology access.
Program Management & Community Engagement
Support the development and implementation of wellness programs focused on chronic disease prevention and management.
Work collaboratively with healthcare providers, community organizations, and social service agencies to enhance program effectiveness.
Plan and coordinate wellness workshops, support groups, and educational events.
Promote program awareness through outreach efforts and engagement initiatives.
Data Tracking & Reporting
Maintain accurate documentation of member interactions, progress, and program participation.
Track program success metrics and provide insights for continuous improvement.
Generate reports and contribute to program evaluations to measure impact.
Qualifications
Education: Bachelor’s degree in public health, social work, health education, psychology, healthcare administration, or a related field (or equivalent experience).
Experience: 2–5 years of experience in community health, wellness coaching, patient advocacy, case management, or a related non-clinical role.
Strong understanding of chronic disease management principles, particularly for diabetes and hypertension.
Excellent communication and interpersonal skills with the ability to engage in diverse populations.
Experience working with underserved communities and addressing social determinants of health.
Proficiency in data entry, Microsoft Office, and case management software.
Bilingual skills (Spanish or other languages) are a plus.
